Criticisms of care providers, the DHSS, the police, and politicians dominated proceedings at the childcare inquiry today.
Chairman Mark Everall QC produced more than 130 recommendations after hearing evidence about the lives and deaths of George Green and Samantha Barton (pictured).
They were murdered by Peter Newbery in February 2002, and the inquiry was set up to investigate the care system to prevent similar tragedies in future.
Mr Everall said there had been systematic failures by senior management at social services, and mistakes had been made by frontline care workers.
He also said police should have charged and held Newbery over the attempted rape of a teenage girl the month before the murders, which would have saved the lives of George and Samantha.
The families’ legal representative at the inquiry, Chris Gawne, said both the Greens and the Bartons welcomed today’s findings:
